I also was noticing that late Ascendant, which suggests that for some reason, it is a bit late for making this arrangement.
I was thinking that Mars, ruler of Asc, is approaching Venus, ruler of 6h of surgery. As Venus and Mars are some 12degrees apart, it could be that in 12 units of time (months, possibly?), the surgery takes place.
Yes, Saturn could be creating the delay that creates this much lengthier time-scale. Also, Saturn is co-ruler of 2h of money, so that could play a part. Obviously, I don't know the financial ins and outs of your situation (and I would not be prying, either), but if it is a state-run hospital that would carry out the surgery, it could be about their funds, rather than your personal funds (if it is a state-run outfit, this might also account for the late Ascendant - if the hospital has an issue with funding, then they might already be pushing everything back in the calendar. As Saturn denotes age also, I wonder if surgeons with the requisite skills might be in short supply - retirement affecting available talent, for example?).
Mars is in Saturn's degrees of Aquarius, so Saturn does play a strong role here in the question.
